Join our dedicated team as you help strengthen the connection between patients and their primary care providers while promoting overall health and well-being. In this role, you'll engage with patients to help them navigate their healthcare journeys, resolve any issues they face, and enhance communication with their medical practices.

What You'll Be Doing

Collaborating remotely with patients' medical practice staff and our Case Management team to manage a targeted patient population.
Functioning independently while also being a valued team member, acting as an extension of the medical practice.
Assisting patients in achieving better health outcomes by addressing their questions and concerns, connecting them with their medical practices, implementing impactful care plans, and closing care gaps. You'll also escalate medical issues as necessary and guide patients to the appropriate resources, including specialty care and community-based organizations.
Accurately documenting patients' medical histories and issues in the electronic health record (EHR).
Understanding the referral and prior authorization processes to facilitate patient care.
Building and maintaining trusting, professional relationships with assigned medical practices and within our organization.
Meeting assigned Key Performance Indicators to foster positive patient health outcomes while minimizing medical expenses.
Engaging high-risk patients through effective communication methods, including calls, texts, portals, and emails.
Appropriately escalating medical concerns to the Health Guide Registered Nurse, Primary Care Physician, or leadership.
Occasionally traveling to support primary care physician offices.
Adhering to all security policies and procedures to protect all Protected Health Information (PHI) and company intellectual properties.
What You'll Need to Have

Bilingual (Korean/English) - Native or fluent proficiency is required.
Certification as a Medical Assistant or as a certified EMT is required.
A passion for patient coordination and a commitment to serving individuals with complex medical and social needs.
Empathy and a willingness to develop skills in motivational interviewing, critical listening, and behavioral change management to enhance patient engagement.
Exceptional communication skills and the ability to document patient interactions accurately and efficiently using software.
Familiarity with medical terminology and the functioning of primary care practices.
Knowledge of social determinants of health.
Proficient in documenting within electronic health records (EHRs) and using care management analytics databases, along with various software programs.
Strong ability to balance competing priorities while remaining detail-oriented and focused.
Familiarity with various electronic health records systems such as EPIC, Athena, and Cerner.
Excellent written, visual, and verbal communication skills.
Ability to track multiple tasks and ensure timely completion.
Capability to work independently as part of a patient's overall care team, under the supervision of licensed clinical professionals.
Proven success in a remote work environment.
Additional Information

The base pay range for this role is $24+ per hour, paid bi-weekly.

The compensation range is based on our standard payroll practices and may vary based on factors such as skill set, years of experience, education, location, and certifications.
