We are a dedicated and patient-focused Home Health agency looking to expand our team with a caring and skilled Home Health LVN or RN Nurse Office Case Manager. In this role, you will assess, diagnose, and treat assigned patients while collaborating closely with our field Nurses who directly interact with patients.

Requirements:

New Grads Welcome
Valid CA Nursing license
Excellent customer service and interpersonal skills
Strong problem-solving and critical thinking abilities
Ability to thrive in a fast-paced environment
Excellent computer skills

Collaborate with Field Nurses to diagnose and treat patients
