OPEN JOB: Inpatient Social Work Case Manager - LMSW/LCSW-C
LOCATION: Baltimore, Maryland - on-site is required
SALARY: $62,400 to $83,200
BENEFITS - Full
BONUS ELIGIBLE - Yes
OVERTIME ELIGIBLE - Yes

• Will accept LMSW or LCSW-C.
• Must have prior experience with discharge planning in an inpatient/hospital setting.
• This position works with general MEDICAL inpatient population - THIS IS NOT A MENTAL HEALTH/PSYCH POSITION. 
• The Inpatient Social Worker, in collaboration with the clinical team and medical provider, provides patient and family advocacy, discharge planning coordination, and psychosocial intervention for the high-risk inpatient.
• The Social Worker strives to promote patient and family wellness, improved care outcomes, and access to appropriate hospital and community resources among a patient population with complex health needs.
• ***This team also offers supervision hours for LMSWs wanting to pursue their LCSW. 

Skills Required:

• Master's in Social Work
• LMSW or LCSW-C
• Social Work License in the state of Maryland per level of education
• Prior experience working in an inpatient/hospital setting
