Job Description

We are seeking a dedicated and experienced nurse to join our team as a CCM Team Lead. This is a remote position, primarily based out of California but open to applicants within the PST time zone. As a CCM Team Lead, you will play a crucial role in supporting our nurse team, especially when the team manager is unavailable. You will assist with patient triage, including handling emergent clinical needs, and provide training and mentorship to new nurses managing patients with complex illnesses, recovery from traumatic events, or multiple co-morbidities. Additionally, you will conduct phone and chart audits as directed by the team manager.

Daily Responsibilities
As a CCM Team Lead, your essential functions include, but are not limited to, the following:

Patient Assessment and Triage: Perform accurate and timely assessment and triage of patient calls to identify clinical needs and create appropriate care plans.
Care Plan Management: Develop and manage patient care plans, ensuring all identified issues are addressed.
Preventative Assessments: Conduct telephonic assessments to screen for new issues, reassess patient conditions, update care plans, and evaluate progress toward health goals.
Symptom Evaluation: Monitor changes in patient-reported symptoms, conduct additional screenings, and determine next steps.
Clinical Referrals: Refer patients to their primary care provider or hospital when necessary.
Patient Education: Educate patients and families about treatment options, including cost, efficacy, and potential side effects.
Job Requirements
To be successful in this role, you should possess the following qualifications:

Time Management and Flexibility: Demonstrated ability to manage time effectively, adapt to changing circumstances, and work independently.
Licensing: Active, unrestricted RN license in the state(s) of practice. Multi-state license required.
Experience: At least two years of clinical experience, with a preference for telephonic care management experience.
Technical Proficiency: Proficiency in Microsoft Office programs and the ability to document patient assessments telephonically.
Empathy and Ethics: High degree of empathy and compassion, along with excellent ethical standards.
Analytical Skills: Ability to analyze patient claims data, draw accurate conclusions, and act promptly on the information.
