About the Role:
Position: RN Case Manager (70% In-Person, 30% Remote)
As a Registered Nurse with us, you'll play a crucial role in developing and implementing care plans for children with IDD. Your responsibilities will include:
Conducting in-home visits every 60 days to assess patient needs and progress.
Collaborating with families to create comprehensive care plans and providing skilled nursing care.
Educating and supporting parents on their child's health needs through coaching and telehealth.
Using telehealth tools to enhance communication with the care team and families, ensuring continuity of care.
Requirements:
Active Registered Nurse license in Colorado or compact state, with at least 2 years of nursing experience.
Experience in home care, with a preference for those skilled in training family CNAs.
Ability to thrive in a fast-paced startup environment, based in or willing to relocate to Fort Collins, CO.
CPR certification, BLS certification required; PALS certification preferred.
Pediatric nursing experience highly desired.

Daily Schedule:
Monday: Mandatory nurse meeting in Denver office from 9am to 10:30am; remainder of day for remote administrative tasks.
Tuesday-Friday: Conduct in-person recertifications (up to 3 per day) and start-of-care assessments (up to 3 per week); balance of time allocated for remote charting, family communications, and case management.
